Insurance plans vary greatly. For this reason, we ask that you be aware of what your insurance plan covers, particularly deductibles and co-payments. We participate with Blue Cross / Blue Shield, Medicare, and several other third-party payors.
Please check with your insurance carrier about whether office visits are covered by your plan. If you’re covered and our office participates with your insurance, bring your insurance card to your appointment. We will gladly submit all claims for you.
If you have a co-pay or are not covered, or if we do not participate with your insurance carrier, we would appreciate payment at the time of your visit. We accept Visa, MasterCard, Discover, American Express, cash, and personal checks. Please notify us if your insurance coverage changes.
